By registering for this Program (workshop, camp, event, session, etc), and in consideration for the opportunity to participate in the Program, I hereby release, indemnify and hold harmless on behalf of myself, my child, our respective heirs, representatives Re-Deme Studio, its officers, agents and employees from any and all causes of action, claims, or demands of any nature whatsoever, including but not limited to a claim of negligence, which I may now have or have in the future against Re-Deme arising out of or in any way related to my or my child’s child’s enrollment, or inability to enroll, in the Program.
Through my signature below, and checking the appropriate box during online registration, I acknowledge that I have read this Release and Indemnification and understand that it includes an assumption of the risk and a release of liability. I understand that Re-Deme is relying on this waiver in allowing participation in the Program.
For child registration:
I hereby certify that my child has no limitations that with or without a reasonable accommodation would preclude his/her safe participation in the Program.
